What the system is
Make, model and vintage if you have them. A photograph of the panel or the rack is worth several paragraphs.
What changed
Whether the system ever worked, and what happened around the time it stopped. Most faults have a date attached to them.
What the constraint is
A shutdown window, a performance date, a budget cycle, an inspection. Constraints shape the approach more than the fault does.
